You must complete the application yourself.  No one else should complete or write on the application,FAILURE TO PROVIDE ALL REQUIRED INFORMATION OR FOLLOW INSTRUCTIONS MAY CAUSE YOUR APPLICATION TO BE REJECTED.

INCLUDE A COPY OF THE FOLLOWING DOCUMENTS WITH YOUR  APPLICATION:

1.    Valid Drivers License  (Picture I.D. will not do)

2.    Social Security Card

3.    Birth Certificate (not necessary if you furnish 1 & 2)

4.    If you have one – any United States Coast Guard Document of License
